You have the option to use the CPAP supplier of your choice. We are in-network with most major insurance plans. We will seamlessly facilitate the transfer of your CPAP re-supply orders, so you don’t have to.

To process the transfer of your on-going CPAP re-supply orders, we require the following information:

Name, phone # and email address, and DOB
Copy of your insurance card
Physician name and phone #
Name of CPAP machine and mask (with size), if available

Once received, we’ll request a copy of your sleep study and prescription from your physician.

We’ll contact you every 3 months with your chosen communication method to confirm re-orders.
